Login to your Baylor Scott & White My Chart account to access your health data on mychart.sw.org. If you need to get one of your prescriptions refilled but do not have time to make an appointment and see your doctor, you can send in an electronic request to your health team to refill your prescription. You can do this from the convenience of your own home computer, or on-the-go from the MyChart Mobile Application, which is free for download in the Apple and Android App stores. Logging into your MyChart Scott and White Account 
The first thing that you will need to do is create an account using the activation code that you have been provided with. This should have been on your visit summary sheet. If you have not yet been provided with an activation code, you will need to call your physician’s office to ask them to provide you with one. If you need assistance with your account you can call 855-691-0180.
If you have already set up your account and would simply like to log in to access it, you just need to provide your login information into the fields provided. This login information includes your MyChart Username and password. If you happen to forget the username or password for your Scott & White MyChart account, there are a couple of links available to help you retrieve or reset this information.
