Access your account with SwedishAmerican by logging onto your MySwedes Chart on myswedeschart.swedishamerican.org. SwedishAmerican is based out of Illinois and currently serves the individuals, communities, and families of northern Illinois and the southern area of Wisconsin. This health system is a division of UW Health.
SwedishAmerican serves people through a variety of different facilities, including: a major hospital, a Medical center, a nationally recognized free-standing outpatient cancer-center and a large network of more than 25 primary care and multi-specialty clinics. Logging on to your MySwedes Chart Account 
If you already have an account with My Chart Swedes, then you can easily log in using your personal Username and Password and entering them into the appropriate boxes on the home login page. You can tick off the “Remember me” box if you are accessing this page from a secure personal computer and would like to save your login information to your computer to make logging in next time even faster.

Keep in mind that you will need to provide a selection of basic personal information in addition to your patient code. It is wise to have this information handy before you being the sign-up process.

Keep in mind that you cannot leave the window dormant for very long because the security system that is in place to help protect your account will notify you that you have about 30 seconds to activate the page again or you will be automatically logged out and will need to enter your login information in order to log in again.
