View your electronic health record online and at your convenience at mychart.nm.org. All you have to do as a patient of Northwesten Medicine is log into your account and you will be able to review an electronic version of your personal health record. You will also be able to send secure electronic messages to your doctor’s office and access other important information that is related to your health care.

Logging into Your NMMyChart Account 
If you would like to log into your N M MyChart account, you can do so by entering your username and password into the secure login area. If you have forgotten your username or password, you can use the two links provided at the bottom of the secure login area. These two links will allow you to restore or recover your information easily.
If you do not yet have an account, you can use the I Have an Access Code button, in order to create a brand new account for yourself. What if you do not yet have an access code? Well, an activation code is necessary for the activation process. You can use the I Don’t Have an Access Code button in order to start yourself on the process of getting your own access code.
After logging in you will be able to do several things that have been made available for your convenience. These include:
