If you are a patient of Deancare that is looking to view your electronic health record or communicate electronically with your care physician or medical care team, you can do so my logging into your mychart.deancare account. You can even access the features of MyChart from your smartphone or tablet by downloading the MyChart application from the App Store or the Google Play store. If you would simply like to log into your account you can do so by entering the proper information and clicking the Sign In button.

If you are a patient that already has MyChart and would like to review the Terms and Conditions that you agreed to when creating your account, you can do so by clicking on the Terms and Conditions link on the homepage. You can also use this link along with the other two links provided if you want to sign up for a MyChart account. The “Learn more about MyChart” link is particularly helpful for those interested in activating their own personal Deancare My Chart account, because it lets you know about the variety of different features available to you by having an account. These include features such as:
- Reviewing your clinical lab results
- Reviewing instructions given to you by physicians from a recent hospital visit or emergency or from a lab result
- Request to make a new appointment
- Getting appointment reminders
- Review your allergies
- Set up MyChart accounts for your children
- Review summary reports from recent visits to your clinic physicians
If you already have an account you can log in using your MyChart ID and password. If you happen to forget either your username or your password, there are two links located near the fill-in boxes on the homepage that you can use to get the proper assistance. On a similar note, if you want to sign up for an account or use your activation code to set up your account, there are two yellow buttons on the homepage that you can use to redirect you to the proper page as well.
