You can now access your MyChart Franciscan account with more ease than ever by downloading the MyChart mobile application! You can access a quick link to the download page of the app on the App Store or the Google Play store.
If you already have your account information programmed into the MyChart system, you can log in easily by entering your unique login details on the home login page. This information includes your MyChart Username and Password.
If you do not yet have this information entered into the system and would like to get your account set up, you can do so by clicking on the “Sign Up Now” button on the login page. Activating this link will take you to a new page that will ask you to provide your activation code, social security number, and date of birth. If you do not yet have an activation code, you will need to request one from your physician.
