It is important to note that depending on which physician you have, they may not take part in all of the features that Henry Ford My Chart has to offer. Because of this, you should check with your physician to ask how you might interact with them using MyHenry Ford Chart. You should also be sure to read through the frequently asked questions page to get more information on the program.
Once you are ready to create your MyChart HenryFord account, you will need to use the Sign Up Now button in order to do so. Activating this button will redirect you to a new page that will ask you to identify yourself as the first step of the account creation process. In order to fill out all required field on this page you will need the following information:
- Your MyChart Activation Code (use the green button at the bottom of the page to request an activation code if you do not already have one)
- Your Medical Record Number (an 8-digit number)
- Your Date of Birth in mm/dd/yyy form
Once you fill in this information you will be able to move onto the next step.
After you have created your username and password, it is suggested that you keep both in a secure and private place so that you do not lose them.
