Communicate with your doctor and health care team by logging into your MyChart Reliant account on mychart.reliantmedicalgroup.org. By logging into or creating an account you will be able better manage your health by using any or all of the helpful features that your account comes pre-loaded with. The MyChart program is offered to patients of Reliant Medical Group and SouthBoro Medical Group at no cost. Using the program will give you an internet based connection with your doctor-s office and to parts of your real medical record. You will also be able to schedule appointments, cancel them and review portions of your health history, all from the comfort of your own home.
You can be absolutely certain that all of your information is completely secure with MyChart. The MyChart website is certified as secure by “Security METRICS Credit Card Safe”. Keep in mind, however, that the MyChart program, including the communication feature, should never be utilized for urgent or emergency medical situations. If the medical problem that you are encountering is of an urgent nature please call your physician’s office or 911. Navigating MyChartReliantMedicalGroup

Reliant Medical Group is a company that is dedicated to delivering care that meets your medical needs. You can log into you’re your account by providing your MyChart Reliant ID and Password. If you have forgotten your user id or password, you can use either of the two links available to help you recover this information.
If you are new to Reliant My Chart and need to set up an account, you can use the button labeled “Create a MyChart ID” in order to begin the process. Activating this button will redirect you to a new page that will require you to fill in your MyChart Access Code. If you do not already have an access code you can request one using the button on the login page labeled “Request Access Code”.
